The Coordinated Service Planning program is here to simplify the journey. We don’t provide direct treatment, but we do something just as important: we provide a single, dedicated coordinator to help your family access the right services, in the right order, all in one place.

Who We Help

Coordinated System Planning is designed for children and youth (up to age 21) with multiple and/or complex special needs who are involved with several different services or agencies.

How It Works: One Coordinator, One Plan, One Team

Your Single Point of Contact: You will have a dedicated Service Planning Coordinator who gets to know your child and family. They will act as your single point of contact, simplify communication for your family and all involved service providers, and will ensure all the services in your child’s life are working together as a unified team toward a common goal.

One Unified Plan: We develop a single, integrated plan that brings all your child’s services and supports together, eliminating confusion and overlap. We actively connect you to the right community resources and clinical supports, ensuring there are no gaps or overlaps in your child’s care plan.

A Collaborative Team: Your coordinator works with you and all involved professionals (like therapists, educators, and health providers) to ensure everyone is working toward the same goals.

These needs often involve a combination of mental health, developmental, behavioural, and/or physical health conditions that require coordinated support from several different service systems.

Eligibility

As this is a program funded by the Ministry of Children, Community and Social Services (MCCSS), and all referrals must come through Contact Hamilton.
